Records of qualified professionals reviewed and certified to provide the services as a Hygienist to collect and interpret samples or to provide professional services to clean up of these sites to render safe for human use after a CML event.

The files are to be cut off at the end of each 2 year permit period. Paper records are maintained in agency until they are converted to electronic and microfilm, within one year of cut off. Paper records are destroyed after verification of electronic and microfilm record. The microfilm will then be stored at the State Records Center permanently. Electronic record will be maintained permanently in agency for administrative purposes.
