Record Series consists of the following: a). Discharge order and, if applicable, amendment order(Note: Discharge orders are not published on a soldier who is discharged b). Current NGB Form 22 (Copy 4) c). Prior Service NGB Form 22 d). Any DD Form 214, DD Form 214, DD Form 220, or Active Duty Letters in lieu of a DD Form 220 e). Last promotion and/or reduction order showing the highest rank held. Officer files must also contain a copy of both the state and federal recognition. f). Last prepared NGB Form 23 g). Copy of closed out DA Form 2-1/ERB/ORB h). Copy of 20 year letter for those going to Retired Reserves i). Copy of DD Form 1883, Survivor Benefit Plan j). Any Line of Duty Investigations (LOD) k). Enlistment waivers for previous and current enlistment (DA Form 4386, 4-1 Series). l). Officers only: copy of federal recognition withdrawal if available.
